OBTAIN A REPEAT PRESCRIPTION?
With each prescription
you will receive a tear-off slip which lists all the items you have been prescribed.
You should return the slip to the surgery (with a SAE if you wish it to be posted)
indicating, with a tick, which items you require. Please allow 48 hours (not including
weekends and holidays). We regret that we CANNOT
take repeat prescriptions over the telephone unless it is URGENT or the patient
is housebound. Prescriptions may be collected during opening hours, Monday to
Friday.

NON-NHS
CERTIFICATES?
Sickness certificates covering the first seven days of work are not provided by
the doctor. Self-certificates for this purpose are available from reception or
your employer. Other certificates or forms, such as passport applications, holiday
cancellations, bank loan completion forms and disability allowance forms will
be completed by the doctor and a fee may be payable.
